MEMBERSHIP DETAILS:

• Monthly membership fees and membership registration fees are not able to be claimed through insurance; however, all other treatment fees are able to be claimed under insurance.
• Monthly membership fees will be automatically paid by the provided credit card on file through secured jane payments.
• Memberships are not available for online purchase; however, once purchased you can schedule your appointments as you normally would, and the price will auto adjust to the member price.
• Memberships are not transferable.
